Our coordinating site (The Walton Centre, Liverpool) send a batch of blank questionnaire packs to participating study sites

Step 6

Site sends a batch of completed questionnaire packs back to co-ordinating site where date of delivery is recorded

Step 7

Anonymous Data is entered into a secure password protected database by a trained member of the study team. Personal identifiers are not entered and are stored separately

Step 2

The site anonymise questionnaire pack by adding a unique Study ID number and date of dispatch

Step 5

Site receives pack from participant and completes a complete check for missing or erroneous data

Step 8

Every 10th questionnaire pack entered is checked by an additional study team member. Where any errors are recorded, data is corrected.

Step 3

Sites send questionnaire pack to participant to complete at home. A prepaid envelope is included for the participant to return the completed pack

Step 4
